Dr Hall is registered to render Ezyclaim accounts. A co-payment of $200 for day procedures such as rhizolysis will apply to be paid prior to surgery. A co-payment of $500 for open procedures will apply, to be paid prior to surgery. The co-payment for complex procedures may be higher and will be discussed at consent for surgery. Co-payments are not covered by your private health insurance or by Medicare. If you are insured by NIB, then the gap will be greater owing to their lower rebates and will be discussed with you prior to surgery.

Dr Hall has an Assistant at most operations. You will receive a separate account from the Assistant but no gap. Please note Professor Freeman's Assist account will be issued from the Royal Adelaide Hospital. You will need to submit the account to your private health fund and to Medicare and then send the two cheques to the Royal Adelaide Hospital for payment.

The Anaesthetist for all procedures will be Dr Mark Williams from Stace Anaesthetics (unless he is on leave). Dr Williams' rooms will contact you prior to surgery, but he will either charge you no gap or a minimal gap of $100 or less.

There should be no other gaps other than those listed above.
